These Terms of Service govern your access to and use of the Zaltivex platform, including the website located at zaltivex.com and all associated care coordination services provided by Zaltivex Dental Solutions. By accessing the platform or submitting information through any Zaltivex form, you agree to be bound by these terms. If you do not agree, please discontinue use of the platform.
Zaltivex provides care coordination, patient navigation, and provider matching services in connection with dental health care. Zaltivex is not a dental practice, does not provide clinical dental services, and does not employ licensed dental clinicians in a direct patient care capacity.
Care recommendations, provider referrals, and clinical information communicated through Zaltivex are intended to facilitate access to qualified dental professionals and do not constitute a patient-provider relationship with Zaltivex or Zaltivex Dental Solutions. All clinical decisions and treatment recommendations are the responsibility of the licensed dental providers to whom patients are referred.
By using the Zaltivex platform, you agree to provide accurate, complete, and current information in all intake and contact submissions. You acknowledge that the accuracy of care coordination recommendations depends substantially on the accuracy of the information you provide.
You agree not to use the Zaltivex platform for any unlawful purpose, to misrepresent your identity or health information, or to interfere with the operation of the platform in any manner.
All content on the Zaltivex platform, including text, design elements, logos, and coordination frameworks, is the property of Zaltivex Dental Solutions and is protected by applicable intellectual property law. Unauthorized reproduction, distribution, or modification of Zaltivex platform content is prohibited without prior written consent.
Zaltivex and operated by are not liable for clinical outcomes resulting from treatment provided by referred dental professionals. Care coordination services are provided to facilitate access to qualified providers, and the clinical relationship and associated liability remain between the patient and the treating dental professional.
To the maximum extent permitted by applicable law, Zaltivex Dental Solutions shall not be liable for indirect, incidental, consequential, or punitive damages arising from your use of or inability to use the Zaltivex platform.
By enrolling in Zaltivex SMS communication programs, you agree to receive text messages from Zaltivex as described in your consent elections. You may withdraw consent at any time by replying STOP to any Zaltivex message. Standard message and data rates may apply. Zaltivex will not share your mobile number with third parties or affiliates. Enrollment in SMS communications is not required to access Zaltivex services.
Zaltivex reserves the right to modify these Terms of Service at any time. Continued use of the platform following the posting of updated terms constitutes acceptance of those revisions. We encourage you to review these terms periodically.
These Terms of Service are governed by the laws of the State of Nebraska, without regard to conflict of law principles. Any dispute arising from these terms or your use of the Zaltivex platform shall be subject to the exclusive jurisdiction of the courts located in Douglas County, Nebraska.
